Clients are provided with written intake and evaluation services by the outreach worker to determine their needs. This assessment service can be performed at the Central Outreach Center for walk-in clients or at other institutions or areas for clients who cannot be present at the office. This begins the process through which services are provided to the client.
Case Management services and counseling are provided to ninety percent (90%) of clients interviewed at the Outreach Center to in order to ensure that they have access to mainstream services. Eighty (80%) of clients referred to mainstream programs are assisted in receiving services such as food stamps, medical assistance, mental health counseling and temporary or permanent housing. Tracking is also done through weekly visits to the clients by our case managers and outreach workers.
After the intake and assessment process is completed, clients who are in need of other services are referred to providers such as the
Case Managers also assist clients in finding permanent housing through the local landlord market.
Case managers at MTOC perform counseling and assessment with homeless clients who are in need of job skills development and training. Referrals are made to Department of Labor Work Study Program, Job Training, Department of Human Services, or Work Able Program for further assessment, development and training in order to enable them to acquire skills for employment.
Through a grant from the Department of Housing & Urban Development, MTOC, and V.I.Care of St. Croix is able to provide a continuum of housing opportunities, from emergency shelters to permanent housing for persons with HIV/AIDS. Services include hotel vouchers, short term rental assistance, utility assistance, permanent housing through master leasing for individual and families, education counseling to improve participants' independent living skills, and referrals to existing transitional and permanent housing services, as well as other related social services.
